Operator understands that the RVSM Representative designated on this form must be the first point of contact for the FAA. Any technical questions or changes to the RVSM manual requested by the FAA can be directed to Business Jet Consultants, LLC until the LOA is issued.

The charge for this service (including support through the approval process) will be discussed after form is received and is due prior to shipment of the RVSM manuals.
